【Abstract】Objective To evaluate the clinical significance and effect of the anomalies seen at the four-chamber view antenatally in diagnosis of fetal congenital cardiac malformation. Methods Seventy-one cases with congenital cardiac malformation(s) were diagnosed on prenatal ultrasonography.We classified them into three groups according to the difference of provided clinical diagnostic significance for antenatal diagnosis of fetal congenital cardiac malformations based on the anomaly seen on the four-chamber view. ResultsFifty two cases (52/71, 73.24%) in group 1 could be diagnosed definitely or partially according to the features seen on the four- chamber view, but all the large vesselsmalformations 25 cases (25/52 ,48% )in this group were missed diagnosis if only using the four-chamber view. Twelve cases (12/71, 16.90%) in group 2 presented important clues which may obviously suggest the presence of some diseases on the four- chamber view. Seven cases(7/71, 9.86%) in group 3 showed normal features on the four- chamberview, although they carry the disease in each. In the mean time, 39 cases (54.52%) were found incorporating extracardiac malformations in the 71 case. ConclusionsIt had real important clinical significance and effect that using four-chamber view antenatallyUltrasonography screening be done for fetal congenital cardiac malformation. The majority of congenital heart anomalies (over than 90%) could be demonstrated abnormal configurations at four-dimensional view by various levels, although it is frequently less sensitivity to identify aortic and pulmonary
